APPOINTMENTS
Prior to your first scheduled appointment, you will receive an intake packet. These forms can be returned via email or in-person on the date of your first appointment.
Initial intake appointments are 60 minutes. This appointment provides an opportunity to assure therapy with Leslie is a good fit and to plan for follow-up appointments. Individual therapy sessions can be 45 minutes or 60 minutes. Follow-up couples therapy sessions can be 60 minutes or 90 minutes.
Leslie offers a free 15-minute phone consultation before the start of therapy to further discuss these options.
FEES AND PAYMENT
Leslie Sickels does not accept insurance or insurance payments.
Intake sessions for individuals and couples are $250 (1 hour)
Individual sessions are $190 (45 minutes) or $250 (1 hour)
Neurodiverse couples sessions are $250 (1 hour) or $375 (1.5 hours)
Group/dyad session fees vary based on content and length
Payment can be made in the form of a check, cash or credit card. Leslie cannot accept any payment or checks from insurance. If you are paying by credit card, the card you put on file will be charged after each appointment. If you are paying through check or cash, payment is due on the date of service unless another payment arrangement has been agreed upon.
Each month you will receive an invoice reflecting all billed appointments. If your insurance has out-of-network benefits, you can submit this invoice to your insurance company for reimbursement. If you plan to pursue insurance reimbursement, it is best to contact your insurance company prior to your initial appointment to discuss your out-of-network coverage and your reimbursement rate. They may request my NPI #: 1235404302 and the CPTs codes for services. The most common CPT codes used for billing are:
Individual therapy, 45 minutes: 90834
Individual therapy, 60 minutes: 90837
Couples therapy, 60 minutes: 90847
You are financially responsible for any appointment canceled less than 24-hours in advance of your appointment time. It should be noted that insurance will not reimburse for missed appointments.
